These questions need to be answered for each unit that is to be equipped with the applications for the Atomization Systems, and Deep Pit Injection. If multiple buildings are in a row or are seperated by more than one row, a drawing will need to accompany this form.

Type of building, ie. tunnel, open front, etc.
Building used for, ie. Nursery, Finishing, etc.
Building Dimensions L x W
Manure handling, ie. Pit, scraper, flush
Wiring on farm, ie. Underground or overhead
Utilities located around facilities, ie. Phone, water, gas supply, electrical, etc.

With the above utilities you have listed, is there an area that would be free of these sources for underground runs of piping to each building that you are wanting to treat? Yes     No

If No to the above question, we would need to have the hazards indentified and marked by precise instruments, to insure no damages or loss of service to the facilities that are being equipped. If these services are not known by the operations, then it will become the responsibility of the operation to have the agencies or suppliers come to the farms to identify where the equipment is located respectively.

How many buildings are to be equipped with the systems?
What is the distance between buildings?
Are all the buildings the same size?
If different sizes, indicate how they are positioned in respect to a straight line.
With this information you have provided we will want to make an on-farm assessment of the data you have provided for final engineering and applications agreeable by both parties. Please give a detailed set of directions to the site and the name and number of the contact person so the layout may be agreed upon before the final engineering is submitted for pricing.
